Skilled Nursing and Rehab: LCH can transition you from ANY hospital back to home. In many cases, patients are not ready to return home after a hospital stay. They need more time to strengthen and heal before they can step back into the routine of independent living.
Our Skilled Nursing & Rehab Program is a Medicare-certified skilled nursing program that provides patients short-term rehabilitative therapy. Physical and Occupational Therapy: Our highly trained, caring and professional staff will evaluate and develop an individualized treatment program that enables the patient to return to their optimal function needed at work and home. Pulmonary Rehab: Are you suffering from COPD, Emphysema, Chronic Bronchitis or other lung-related issues, give YOUR local hospital a chance for your rehabilitation. The LCH Pulmonary Rehab unit can help you in your time of need and save the drive to bigger towns when we have the same exact service here in Lauderdale County!!! Cardiac Rehabilitation: If you are a patient recovering from bypass surgery, heart attack, coronary interventions (stent), stable angina or valve replacement, no longer do you have to drive to Jackson or Memphis for your rehab. Contact our Cardiac Rehab department to schedule your treatment or to have your physician send a referral. The phone number is 731-221-2501.
Monthly Stroke Support Group: Our Rehab Department hosts a monthly stroke support group the first Thursday of every month with timely topics and other vital information for stroke survivors, family members, friends and caretakers. First Mondays at LCCOA: LCH provides a program the first Monday of every month at the Lauderdale County Commission on Aging. A different speaker each month provides a multitude of healthcare topics. The LCCOA is located at 605 Airport Street in Halls. Call 731-221-2402 for more information.
